We hope that the following directions and information will be useful to you and will assist us in making your first visit more enjoyable. We respect your time and would like to make your visits to our office as efficient as possible.

Center for Infectious Diseases and TravelCare are located at 20 Prospect Ave, Suite 507, Hackensack, NJ 07601. Our number is 201-487-4088. We are on the campus of Hackensack University Medical Center, 5th Floor, Suite 507, of the Mediplex Building. Directions from Route 80 West: Follow Route 80 West to exit 64B. Turn right at light onto Polify Road. Travel North on Polify Road. At second light, turn left onto Essex Street. At first light, turn right onto Prospect Avenue. Center for Infectious Diseases in on the right. We share the parking garage with the hospital.

To ensure that your initial visit goes quickly and smoothly, please make sure that your referring physician has sent the necessary medical records. Our office fax number is 201-489-8930. All medical records should be sent to this number. 

Medication refills and scheduling of appointments are handled during normal office hours.
