Medicare 

We participate with Medicare. We will bill Medicare and your secondary insurance provided you furnish us with the appropriate information. If we do not participate with your secondary insurance, you will be responsible for payment if reimbursement is not received within 45 days of submission. Co-payments, deductibles and non-covered services are the responsibility of the patient and payment is expected at the time services are rendered in the office. 

Other Insurances and Managed Care

Care is provided directly to you and you are responsible for payment of services rendered. We require payment be made at the time services are rendered in the office. A form will be provided for you to seek reimbursement from your insurance company. 

Participating Insurances

If we participate with your insurance company, we will file your visits for you. You are responsible for co-payments, deductibles and non covered services at the time service is provided. Should your insurance require a referral or authorization, you will be responsible to provide the proper forms prior to your visit.

Hospital Services

If our physicians provided care to you in the hospital, your insurance will be billed regardless of our participation agreement. If we do not participate with your insurance, you are responsible for payment if reimbursement is not received within 45 days of submission. Patients are still responsible for all co-payments, deductibles and co-insurance. Should you require care in our office, after your hospitalization, you will need to follow your insurance rules, (i.e., managed care companies require a referral). It is necessary for you to check with your primary care physician and/or your insurance company for the rules of your particular plan before your appointment with us is scheduled.
